About the Role

The Community Action Coordinator will work from Access Alliance’s east location - AccessPoint on Danforth Hub and a new Population Health Clinic. This role involves planning, implementing, and evaluating community engagement, peer outreach, and related initiatives with an anti-oppression health promotion focus. A key function is supporting clients with system navigation to access health, social, and community services.

Responsibilities

  • Work with marginalized community members to identify health priorities and implement strategies for better health, focusing on peer outreach and community ambassador models.
  • Collaborate with interdisciplinary teams and community groups to identify needs of immigrants and newcomers, developing or strengthening programming for underserved communities, families, and youth.
  • Utilize diverse community engagement and outreach strategies, including health education and community development.
  • Act as a resource for community groups by facilitating workshops, providing consultation, referrals, and information, and advocating for groups.
  • Mobilize community resources to enhance capacity for social change by developing self-help networks, building coalitions, and fostering leadership.
  • Maintain familiarity with community services and programs, sharing information and developing resources.
  • Develop community-based projects responsive to needs using a project management approach.
  • Generate stakeholder ownership in project planning, delivery, and evaluation.
  • Lead project implementation, including budget development, activity management, and providing guidance to staff, students, and volunteers.
  • Evaluate programs to ensure objectives are met and recommend operational changes based on community input.
  • Initiate, carry out, participate in, or monitor community-based health needs research.
  • Develop links with stakeholders to promote the Centre as a learning site.
  • Participate in committees and work groups.
  • Identify the need for or participate in the development of policies and protocols.
  • Develop an annual work plan with supervisor, identifying action plans and professional development needs.
  • Submit program statistics and reports on a timely basis.
  • Participate in the development of funding proposals.
  • Contribute to the vision, mission, values, principles, and strategic directions of the Centre.
  • Carry out other assigned duties.

Requirements

  • Graduate degree in Health Sciences, Social Work, Immigration and Settlement Services or related field, or a combination of relevant education and work experience.
  • Minimum 3 years of front-line experience in community engagement-based program coordination, planning, implementation, and evaluation.
  • Thorough understanding of health promotion and community development principles.
  • Experience working with low-income and other priority groups.
  • Staff, students, and volunteers must be fully vaccinated.
  • A criminal background check will be conducted for this position.

Skills

  • Strong understanding of the social determinants of health and systemic issues affecting low-income, multi-lingual, and racialized communities.
  • Success in partnership development and funding proposal writing.
  • Experience managing project budgets.
  • Good interpersonal, communication, and teamwork skills.
  • Ability to speak fluently in a language of one of the priority groups served (Arabic, Tigrinya, Dari/Pashto, Bengali or other languages).
  • Prior involvement in community-based research.
  • Experience mentoring staff, students, or volunteers.
