About the Role
The Case Manager is responsible for providing supportive case management services to residents referred by the New York City HIV/AIDS Services Administration (HASA) in a new permanent supportive housing program for homeless individuals and families living with HIV/AIDS. The program aims to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being.
Responsibilities
- Maintain a caseload and provide supportive case management services.
- Maintain resident records in compliance with agency and funding standards.
- Conduct and review assessments.
- Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency.
- Monitor and ensure residents are linked to healthcare providers.
- Provide counseling and crisis intervention as necessary.
- Assist residents in securing and maintaining benefits.
- Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs.
- Facilitate groups and activities that promote health and independent living skills development.
- Monitor and assist residents with rent payments and budgeting.
- Conduct regular unit inspections.
- Educate residents about approaches to mental health.
- Attend ongoing trainings to enhance the services provided to the residents.
- Assist with completion of reports as needed.
- Other duties as assigned.
Requirements
- Bachelor’s Degree in Social Work or relevant field.
- Minimum of three years of post-Master's experience.
- Working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness.
- Excellent case management skills, including knowledge of benefits in the NYC Metro area.
- Sensitivity to community issues and needs.
- Outstanding interpersonal and communication skills, both oral and written.
- Strong ethical standards, confidentiality, and personal integrity.
- Computer proficiency (i.e., email, Microsoft Word, Excel and PowerPoint).
