About the Role
Provide ongoing support to individuals to ensure a high quality of life by directing, planning, coordinating, and leading the inter-disciplinary team to ensure clinical habilitative services are met and regulatory requirements are fulfilled.
Responsibilities
- Practice and promote the agency’s mission statement in every interaction.
- Oversee the development of a Comprehensive Functional Assessment (CFA) for each individual.
- Supervise and monitor the implementation of each individual’s program plan.
- Serve as an advocate on behalf of the individual.
- Monitor goal, medical, and record books for accuracy and compliance.
- Serve as the primary family contact, addressing concerns and providing information.
- Manage the operation and monitoring of the In-House Day Program.
- Implement all service activities, ensuring supports, safeguards, and protective oversight.
- Create and facilitate meaningful, person-centered, and functional day habilitation activities.
- Develop, implement, and evaluate Day Habilitation Plans.
- Provide essential information for the development and revision of clinical documentation.
- Maintain accurate and timely documentation and records.
- Assist with the orientation, training, and mentoring of program staff.
- Report and document any events/activities that may hinder the health, safety, and well-being of individuals.
- Participate in the development and ongoing monitoring of individuals reviewed by the Human Rights Committee and monthly Psychiatric Meetings.
- Ensure individuals receive and maintain all entitled benefits.
- Serve as a member of the admission/discharge committee.
- Develop and update psycho-social histories.
- Ensure work-related safety procedures are in place and adhered to.
- Report to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect.
- Adhere to Federal and New York State regulations, agency policy and procedures, and training requirements.
- Successfully complete and maintain annual training and any other necessary training.
- Perform other duties as requested by supervisor.
Requirements
- Must be a team player.
- Requires regular and substantial unsupervised or unrestricted physical contact with people receiving services.
- Subject to successful completion of fingerprint/criminal background check.
Skills
- Excellent oral and written communication skills.
Experience Level
- One year experience working with ID/DD.
Education Level
- Bachelor's Degree in Human Services or related field.
