About the Role
Health Quest Home Care is seeking dedicated individuals to join their home healthcare team. This role involves independent work at the top of professional licenses, collaborating with physicians and families to provide care in patients' homes as part of the Nuvance Health Network.
Responsibilities
- Assess physical, emotional, socio-economic, and environmental needs of patients and families.
- Provide physician-ordered services as per the plan of care and communicate with involved physicians and healthcare practitioners.
- Develop, implement, and evaluate the plan of care in partnership with the patient, caregiver, and physician.
- Coordinate services with the patient, family, and others involved in the care plan, providing counseling and education.
- Regularly evaluate patient progress, take prompt action for condition changes, and terminate care when goals are met or per agency policy.
- Participate in the agency's quality assurance, performance improvement program, and in-service training.
- Identify and refer patient and family needs for other home health services when appropriate.
- Supervise homemaker/home health aide services according to guidelines and submit evaluations.
- Fulfill compliance responsibilities, including preparing clinical notes and attending in-service training.
Requirements
- Experienced in a qualified home health agency.
