About the Role
Oversees the day-to-day operations of the care management team providing Health Home and deficit funded services to adults with serious mental illness. Manages a team of care managers who provide comprehensive assessment, care planning, coordination, outreach, crisis intervention, linkage to services, advocacy, and resource procurement. Monitors team performance and ensures program outcomes are met.
Responsibilities
- Ensures program activities are coordinated and service/statistical records are maintained according to federal, state, and agency regulations.
- Audits records for adherence to federal and state regulations and addresses documentation problems with staff.
- Ensures all residents receive appropriate services.
- Schedules, coordinates, and assigns staff activities based on resident and program needs.
- Ensures all Electronic Medical Record (EMR) systems are updated and accurately reflect monitored data, including maintaining entries in AWARDS, SPOA, CAIRS, HealtheLink, Netsmart, and Medent.
- Oversees admission and orientation of new case assignments and ensures timely processing of related paperwork.
- Ensures timely development and implementation of Individualized Service Plans.
- Ensures continuity of clients' entitlements through timely submission of required forms and documentation.
- Oversees care management discharges and ensures timely processing of related paperwork.
- Develops effective working relationships with other service providers.
- Assures timely reporting and review of untoward incidents and events.
- Participates in regular and unplanned case reviews.
- Assures that client rights and dignity are maintained and that resident grievances are processed in a timely manner.
- Ensures program coverage at all times.
- Provides supervision, coaching, and instruction to staff through individual and group conferences and direct participation in service provision.
- Evaluates staff performance.
- Orients staff to Agency and program specific mission, philosophy, policies, and procedures.
- Coordinates regularly with ECDMH, NYSOMH, and Health Home Agencies on case-related issues.
- Schedules and conducts regular staff meetings.
- Assures that staff receives appropriate training.
Requirements
- Master's degree in human services related fields.
- Minimum of 3 years' experience supervising staff providing care to persons with mental illness, chemical dependency, developmental disabilities, or a chronic medical condition.
- One (1) year of paid supervisory/administrative experience in a human service setting with experience supervising a staff of 3-5 individuals.
- Must have use of an automobile and valid driver's license with evidence of a good driving record.
