About the Role
The Tunnel to Towers Foundation is seeking an experienced Social Services Case Manager for its homeless veteran program. This role supports the Social Service Manager and manages a caseload of individual recipients within the Veteran Homeless Program. The case manager will facilitate communication between the Foundation, recipients, and their families, address psychosocial needs, and determine the need for referrals to local support or additional resources.
Responsibilities
- Manage communication between the Foundation, recipients, and family members throughout the acceptance process and follow-up.
- Prepare all necessary documentation for potential recipients.
- Maintain current files on program recipients and document all communication.
- Provide psychosocial advocacy and compassionate support to spouses, identifying appropriate resources for families.
- Ensure ongoing psychosocial needs are addressed through follow-up communication with families.
- Perform administrative tasks including email and phone correspondence, file maintenance, spreadsheet management, and resource research for recipients.
Requirements
- Must be willing to travel within the United States on behalf of the Foundation as needed.
- Ability to handle sensitive and confidential matters with discretion.
- Ability to work independently in a fast-paced environment and multi-task efficiently.
- Must be detail-oriented.
- Knowledge of healthcare and social service system entitlements.
- Must possess effective written and verbal communication skills.
- Must have quick thinking and problem-solving skills.
- Must have organizational skills to multi-task and handle various duties in short timeframes.
- Must have 3 years of social worker experience.
- Research and locate resources and social services for qualifying Veterans outside the established network of coverage.
- Veterans are encouraged to apply.
- Pre-employment screening, including a background check, is required.
