About the Role
Special Touch Home Care is seeking experienced RNs for fee-for-service Registered Nurse positions within their licensed home care services agency. This role involves conducting initial evaluations and reassessments of home care clients, assessing clinical needs, observing caregiver performance, and ensuring caregivers are knowledgeable and competent. The position offers independent work, meaningful client relationships, and a positive impact on patient quality of life.
Responsibilities
- Conduct initial evaluations and reassessments of home care clients in the field.
- Assess clinical needs of clients.
- Observe caregiver performance.
- Ensure caregivers are knowledgeable, competent, and properly trained.
- Perform internal case management and related documentation.
- Provide one-on-one patient interaction and field-based oversight.
Requirements
- Current and valid NYS RN License
- Minimum of 1 year of RN home care field experience required
- Excellent patient care and assessment skills
- Willingness to travel throughout Brooklyn, with a strong focus on Downtown Brooklyn, Flatbush, East New York, and surrounding service areas.
Skills
- Excellent written and verbal communication skills
- Experience with UAS assessment/documentation is a plus
