About the Role
As a Resident & Family Experience Coordinator, you will be the main point of contact for residents and families, building relationships and coordinating care and services that enrich their daily health and well-being. You will contribute to the community culture, focusing on cultivating happiness in daily life.
Responsibilities
- Lead the resident move in, transfer, and move out process
- Collaborate closely with the leadership team to resolve concerns raised by residents & families promptly
- Coordinate resident & family satisfaction surveys and build action plans based on feedback
- Coordinate and facilitate resident care conferences
- Engage community guest speakers to conduct in-service sessions related to long term care and services
- Contribute to the development of quality improvement plans
- Conduct assigned Quality Management activities
- Prepare data analysis and trends on a quarterly basis related to resident/family feedback
Requirements
- You are compassionate and dedicated to meaningful work that supports seniors
- You have completed a College diploma or have equivalent experience
- You have 3 years’ experience in healthcare, social services or related sector
- You are customer focused and detailed oriented
- You can multi-task and collaborate with leaders and team members
- You exercise tact and diplomacy in working with seniors and their families
- You must be legally eligible to work in Canada or have a valid work/study permit
- You must successfully pass a Vulnerable Sector Check
