About the Role
This permanent part-time Clinical Partner - Nursing role supports clients in the Central West/Nepean/Blue Mountains Region, working 3 days per week between Monday and Friday. You will be a valued member of the Home Care Clinical Care & Practice Team, collaborating with a multidisciplinary team to provide tailored services to older adults living at home. Responsibilities include assessing care needs, setting goals, and providing treatment and intervention to help clients remain active and independent at home.

Requirements
- Registered Nurse with current AHPRA registration and practical experience
- Sound knowledge in Aged Care, Inpatient, Community, Home-Based or Rehabilitation or related setting
- Strong communication skills
- Ability to plan and manage time effectively
- Ability to work independently as part of a multidisciplinary team
- Access to a reliable vehicle, valid driver’s license and safe driving record
- Consent to a criminal record check and reference checking
