About the Role
The Clinical Case Manager provides specialized clinical case management services to clients and families, helping them navigate and access essential medical, psychological, and social supports. This role coordinates individualized services to promote sustained well-being and supports clients in building stable lives in their new communities.
Responsibilities
- Conducts intake, outreach, and provides clinical case management services through a short-term model.
- Screens for income supports, benefits, and entitlements.
- Receives case assignments, establishes service plans, reviews progress, and determines case closure.
- May provide counseling for motivational and psychosocial issues in individual or group settings.
- Provides advocacy, crisis intervention, counseling, and problem-solving to enhance self-sufficiency.
- Refers clients externally based on needs or program capacity.
- Delivers services professionally, competently, efficiently, and in a timely manner.
- Cooperates and collaborates with internal and external program staff.
- Develops, implements, oversees, and follows up on treatment plans to meet funder-outlined goals.
- Develops, monitors, and enhances professional relationships with social service, health, and governmental agencies.
- Participates in supervision meetings, team meetings, training sessions, conferences, and planning.
- Establishes and maintains professional and therapeutic relationships within ethical and legal guidelines.
- Other duties may be assigned.
Requirements
- BA/BSW and 3 years of experience in a trauma-informed, behavioral, and/or public health setting, OR Master’s degree in social work, public health, counseling, clinical psychology, or related field.
- Communicate effectively, both verbally and in writing.
- Sit, reach with arms and work with hands to operate computers or equipment.
- Stand, walk, climb stairs, and may need to kneel or crouch to access workspace equipment.
- May occasionally need to lift or move items up to 10 pounds.
