About the Role

Whitney Young Health (WYH), in partnership with Capital Region Health Connections, is committed to providing comprehensive community-based case management services to adults with chronic medical conditions and co-occurring mental health and substance abuse disorders, through Health Home Care Coordination across Albany and Rensselaer counties. Health Home Care Coordinators assist individuals enrolled in the program manage their conditions to achieve optimal health outcomes by developing individualized care plans designed to link individuals to necessary services.

Responsibilities

Develop individualized care plans designed to link individuals to necessary services such as housing, healthcare services, mental health and substance use providers, social services, and community programs.
Assist individuals enrolled in the program manage their conditions to achieve optimal health outcomes.
Provide community-based linkages, referrals, and service planning for individuals with chronic medical conditions and co-occurring mental health and substance abuse disorders.

Requirements

Associate’s degree in human services or related field with a minimum of two (2) year’s work experience in case management or case work with one of the following populations: persons with HIV/AIDS, women, children & families, persons with ID/DD, or Substance Use Disorders.
Valid NYS Driver License in good standing (verified annually), and proof of auto insurance in compliance with NYS mandatory limits and coverage.
Strong written and verbal communication skills.
Provides excellent customer service and demonstrates a high level of cultural competency.
Ability to adhere to strict confidentiality guidelines.
Have a working knowledge of computers, particularly electronic health records and Microsoft Office program.
Bachelor’s degree in a health or human services related field such as psychology, sociology, social work, or nursing, with at least one year of related Health Home Care Coordination work experience in a medical setting (Preferred).
Bilingual (English/Spanish) (Preferred).
