About the Role
This role is for a Regular Full-Time Social Worker within the Women’s College Academic Family Health Team (WCAFHT), reporting to the Executive Director, Environmental Health Clinic and WCAFHT. The WCAFHT is a large Family Practice Unit with extensive staff and interprofessional support.
Responsibilities
- Provide patients and families with emotional support, coping strategies, and linkage to community resources.
- Offer social and financial assistance, disability support options, respite care, and child care.
- Provide support, short-term and longitudinal psychosocial therapy, and counseling throughout women's treatment journeys.
- Conduct assessments and participate in developing treatment plans with team members.
- Coordinate referrals and facilitate access to other healthcare providers and organizations.
- Assist patients and families with developing social and community support systems.
- Develop, implement, and balance individual, group, same-day, in-person, and virtual counseling sessions.
- Conduct patient and family education sessions.
- Work collaboratively with internal and external healthcare team members to ensure continuity of care.
- Facilitate patient transitions across the continuum through discharge planning, patient education, and service coordination.
- Establish caring and therapeutic relationships with patients, demonstrating trust, respect, and honesty.
- Provide counseling, including crisis interventions as required.
- Advocate for patients by mobilizing resources and identifying and addressing gaps in supportive care services.
- Participate in clinical research and teaching initiatives in collaboration with Professional Practice, Research, and Education departments.
- Integrate research into practice to maintain best practice standards.
- Identify and lead quality improvement initiatives to advance the work of the Centre and WCH.
- Assist patients and families in overcoming barriers to accessing care.
- Support and coordinate patient care for marginalized and complex patient populations.
- Support and enable strong community linkage to applicable resources.
- Support and champion quality improvement and best initiatives such as social prescribing.
- Act as a role model and champion for safe, best practices in daily activities to foster safe and exemplary care.
Requirements
- Registration with the Ontario College of Social Workers and Social Service Workers is required.
- Master’s degree in social work is required.
- Minimum of 5 years’ experience in a hospital or ambulatory setting is required.
- Ability to form positive therapeutic alliances and work independently and within a multidisciplinary team.
- Understanding of adult education strategies, teaching and learning principles, and advanced therapeutic communication techniques.
- Ability to develop collaborative working relationships with internal and external stakeholders, including community organizations and hospitals.
- Excellent critical thinking, problem-solving, organization, and time management skills.
- Well-developed verbal and written communication, interpersonal, and teaching skills.
- Proficient with Electronic Medical Record (PSS EMR), Microsoft Word, PowerPoint, and internet navigation.
- Demonstrated commitment to a patient/family-centred care environment.
- Evidence of ongoing professional development, continuing education, and leadership experience.
- Demonstrated good attendance record with the ability to maintain this standard.
- Experience working with marginalized and complex patient populations.
- Experience with case management and community linkage.
- Proven attendance and punctuality.
- Professional behavior and communication that meets the standards of the professional regulatory college or association and Women’s College Hospital.
- Plays a critical role in acting as an advocate for safety and will demonstrate principles, practices, and processes that will optimize a safe environment for all.
- Applicants must reside and be legally eligible to work in Ontario.
