About the Role

The PSW Coordinator oversees, supports, and monitors PSWs to ensure direct personal and nursing care meet the holistic needs of the resident, in accordance with Southbridge's policies and procedures. This position is also responsible for the daily operations of the Personal Support Workers (PSWs), ensuring they have the necessary tools, equipment, and resources.

Responsibilities

  • Under the direction of the Director of Care, ensure compliance with the Fixing Long Term Care Homes Act (FLTCA) and Regulations, Canadian Council on Health Services Accreditation Standards, and Southbridge standards, policies, and procedures.
  • Adhere to established policies and procedures for observing, reporting, and electronically recording resident care information.
  • Provide input into the development and implementation of policies and procedures that align with the home's mission, vision, and values.
  • Orient and mentor new PSWs as required.
  • Provide input to registered nursing staff for the development and maintenance of resident care plans.
  • Ensure PSWs follow established resident care plans.
  • Participate in all required education programming.
  • Provide education and training to PSWs, such as safe lifts and transfers.
  • Ensure professional communication with residents, families, and team members.
  • Complete all scheduled and assigned audits.
  • Complete reports as indicated and per schedule.
  • Ensure PSWs have the necessary tools to perform their duties.
  • Participate in all IPAC requirements and observe IPAC practices by PSWs.
  • Participate in the Health & Safety Team.
  • Conduct PSW interviews, hiring, and performance management.
  • Participate in Continuous Quality Improvement and Risk Management activities.
  • Communicate resident progress or decline to the charge nurse or nursing supervisor.
  • Promote and ensure residents' options and choices for hygiene processes.
  • Ensure resident admission, discharge, and transfer processes are complete per checklist.
  • Complete audits on documentation in POC, including BSO, per schedule.
  • Assume responsibility for personal/professional growth and development.
  • Audit therapeutic tubs per schedule to ensure cleanliness and adherence to cleaning/disinfection protocols.
  • Ensure PSWs utilize equipment and supplies responsibly and complete all checklists.
  • Recognize diversity and protect residents' rights to their beliefs and values.
  • Demonstrate respect for co-workers, residents, and visitors.
  • Contribute to person and family-centered care using compassionate communication strategies.
  • Conduct unit audits for cleanliness and tidiness to ensure safety and advocate for optimal resident quality of life.
  • Timely reporting of all resident and employee incidents/complaints.
  • Maintain privacy and confidentiality at all times.
  • Demonstrate understanding of the Residents' Bill of Rights.
  • Perform all duties in accordance with Infection Prevention and Control standards, policies, and procedures.
  • Ensure resident privacy.
  • Respect resident's property.
  • Advocate on behalf of the resident.
  • Carry out all assigned duties in compliance with applicable Occupational Health and Safety legislation and Company policy.
  • Follow safe work practices as directed by their supervisor and required by applicable Occupational Health and Safety legislation.
  • Participate in all required occupational health and safety training.
  • Use appropriate equipment and wear personal protective equipment or clothing as required.
  • Report any known or identified health and safety hazards or violations to their supervisor.
  • Refrain from any conduct that puts themselves, workers, contractors, or others at risk.
  • Ensure the maintenance of a safe environment for residents, staff, and visitors.
  • Report faulty equipment, use Lock Out-Tag Out, ensure immediate removal from the unit, and contact charge nurse or nursing supervisor.
  • Utilize equipment with caution and in accordance with the Manufacturer's Manual(s).
  • Report all actual or potential hazardous situations to the immediate supervisor.
  • Report any personal injury to the immediate supervisor and seek first aid as needed.
  • Participate in fire safety demonstrations and fire drills and know the facility emergency plan.
  • Respect and preserve the privacy, confidentiality, and security of health care and employee information at all times.
  • Adhere to Code of Business Conduct and Standards of Employee Conduct policies.

Requirements

  • Personal Support Worker (PSW) Program Certificate or Diploma meeting appropriate standards.
  • Knowledge and experience in care planning systems and documentation.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Demonstrated skill in working independently with strong leadership abilities, and as a team player.
  • Working knowledge of WHMIS and Basic Workplace Safety Training.
  • Knowledge of mandatory reporting and whistleblowing policies.
  • Current First Aid and CPR Training.
  • Current Vulnerable Sector Police Check.
  • Up-to-date immunization and vaccination records.
  • Demonstrated record of good attendance.
  • Experience in long term care is required.
