About the Role
The Program Support Specialist at a day program emphasizes proactive strategies and responds to the needs of each individual. This role involves coaching staff on skills in a functional setting, modeling proactive and reactive strategies, and providing programmatic support to the site, staff, and participants while ensuring the health, safety, and well-being of individuals.
Responsibilities
- Provide programmatic support for site, staff, and participants.
- Submit and monitor required documentation to the Behavior Department for individuals needing intensive behavior services.
- Provide suggestions and collaboration for ongoing supports, safeguards, and behavior plans.
- Review training provided by the Behavior Department related to behavior plans with staff.
- Initiate communication and update site management on concerns and report on follow-up.
- Ensure current Behavioral Health Planning and semi-annual reviews are onsite.
- Ensure Informed Consents and Site Based Restrictions are up to date annually.
- Ensure fading plans from MMP’s/BSP’s are followed and monitored.
- Complete and review daily eVero DA Logs, Behavior Tracking, Helmet Tracking, Sweeps Tracking, and Restrictive Intervention Applications (RIA).
- Use collected information to facilitate program planning in compliance with Department of Behavioral Health Services Policies and Procedures.
- Consult with site BIS to present HRC packets for individuals without active BIS support.
- Prepare and submit to PD and attend HRC meetings.
- Consult with BIS to ensure all planning is in place for specific site needs.
- Collaborate with team members including SDS, PD, RN to ensure all scripts are updated and available on site.
- Provide assistance with crisis intervention as needed to support direct care staff.
- Ensure supplies/supports are available as per individualized behavior plan or protocol.
- Attend and participate in general staff meetings, in-service training, monthly support team meetings, and other pertinent agency and community program meetings.
- May attend Life Plan meetings as needed.
- Keep a record of all site in-services and trainings.
- Assist in developing Day Habilitation staff action plans that promote an individual’s valued outcome, independence, and ability to contribute to the community.
- Comply with all agency policies and procedures.
- Perform other duties as assigned.
Requirements
- Ability to meet physical requirements including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
- Ability to lift 35 lbs.
- Ability to lift and transfer individuals manually and mechanically as required by prescribed program plan.
- Ability to move, reposition, and place individuals in equipment.
- Physical agility and ability to react to emergency situations.
- Maintain Required certifications (i.e. SCIP, CPR).
- Valid NYS Driver’s license that meets agency policy.
