About the Role
FOX Rehabilitation is seeking passionate, licensed Physical Therapists to deliver exceptional one-on-one care to older adults in their homes or senior living communities. This role offers autonomy, flexibility, and the opportunity to make a meaningful impact on patient mobility, independence, and quality of life through our Geriatric House Calls™ model. Clinicians can also incorporate telehealth and virtual care for enhanced flexibility and patient support.
Responsibilities
- Provide one-on-one physical therapy to older adults in private residences or Senior Living Communities.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
- Document effectively within FOX’s EMR system.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth / virtual care when appropriate.
Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet physical demands of in-home and community-based care, including lifting, patient transfers, standing/walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear PPE when required.
