About the Role

The Case Associate functions as a Case Planner, providing interventions to ensure client care within the community. This role involves developing action plans, identifying and utilizing resources, and ensuring safe family environments through Evidence Based models.

Responsibilities

  • Perform routine community contacts on behalf of social workers, such as obtaining school and medical reports.
  • Locate and utilize appropriate community resources for clients.
  • Evaluate actual living conditions of clients through home visits.
  • Interpret agency functions to clients and demonstrate concrete help.
  • Provide structure to clients, evaluate and assess their use of independent living skills, and offer guidance.
  • Run groups under the supervision of social workers.
  • Evaluate clients in conjunction with the Sr. Case Planner for individualized treatment programs, including child safety.
  • Understand and individualize the needs of clients, families, and their support networks.
  • Identify appropriate treatment or service providers and facilitate client referrals to internal, external, and community linkage programs.
  • Perform individual and group counseling, adapting to client needs for short or long-term support.
  • Conduct home visits to assess the needs of the family and child safety.
  • Identify and help clients obtain necessary services, including maintaining medical, financial, and social resources.
  • Provide case management to clients.
  • Identify and provide access to appropriate concrete services when necessary or requested and in the client's best interest.
  • Perform community outreach and establish linkages with helpful groups, individuals, or organizations.
  • Provide all necessary notes, progress notes, and risk assessments.
  • Make reports to the State Central Registry when required.
  • Identify and monitor high-risk families on caseload who are at high risk of abuse, neglect, and further system involvement.
  • Maintain case records consistent with JBFCS and ACS standards.
  • Complete agency internal Incident Reports in the Alltrac system.
  • Participate in Family Team Conferences (FTCs).
  • Perform other duties as assigned.

Requirements

  • Basic social work knowledge and understanding of stages of change.
  • Ability to provide in-home child welfare services.
  • Ability to establish rapport and therapeutic relationships with families.
  • Knowledge of Microsoft Office.
  • BA Degree with experience in a community or social group, or at least five (5) years of experience as a case assistant in a community or social service group.

Skills

  • Understanding of the use of family therapy principles (preferred).
  • Understanding of early childhood development and related theories and models of practice (preferred).
  • Strong assessment skills to evaluate dynamics and interactions within the family (preferred).
  • Bilingual (preferred).
  • Knowledge and experience with the Connections system (preferred).
