About the Role
Family Service League is seeking a full-time Case Manager to provide comprehensive care coordination and support to individuals with complex behavioral health, medical, and/or substance use needs. The Case Manager will ensure all program visit and documentation requirements for the Health Home program are met, while promoting client engagement, wellness, and recovery through person-centered services.
Responsibilities
- Conduct home visits, community outreach, and engagement using a culturally sensitive, trauma-informed approach.
- Complete comprehensive, consumer-driven assessments and develop individualized care plans.
- Coordinate and monitor behavioral health, physical health, and social service supports.
- Build and maintain linkages with natural supports and formal resources.
- Manage a caseload of approximately 40–50 clients, providing consistent contact.
- Advocate for and coordinate care across multiple systems: medical, behavioral health, and social service.
- Monitor client progress toward goals, supporting self-management and reducing preventable hospitalizations.
- Collaborate closely with multidisciplinary providers and participate in joint case reviews.
- Maintain detailed, accurate, and timely documentation in the electronic health record (EHR).
- Participate actively in supervision, team meetings, and case conferences.
- Maintain a positive, professional rapport with referral sources.
- Respond promptly to crises or urgent client needs, implementing safety plans.
- Serve as a role model of professionalism and FSL’s mission.
- Complete all required reports and documentation accurately and within established timelines.
- Follow all agency and program policies and mandates.
- Perform other duties as assigned.
Requirements
- Minimum of two years' experience providing direct services to people with Serious Mental Illness, medical conditions, developmental disabilities, and/or substance abuse required.
- Excellent interpersonal and verbal and written communication skills required.
- Strong assessment, engagement, organizational and time management skills required.
- Proficient computer skills, including Microsoft Office required.
- Experience with EHR preferred.
- Valid and clean New York State Driver’s License required.
- This position often requires sitting and working at a computer for extended periods of time, traveling to different FSL offices, community meetings, home visits, etc.
