About the Role
At Heart to Heart Hospice, we provide compassionate care to patients with life-limiting illnesses and their loved ones. We are dedicated to making a difference in the lives of our employees by fostering a supportive team environment and contributing positively to the communities we serve.
Responsibilities
- Direct all daily patient referral and intake operations, including policy establishment and implementation.
- Initiate patient charts in EMR and maintain workflow.
- Maintain census, pending admissions, red flag, journey's end, and care plan meetings on a stand-up board.
- Ensure compliance with all state, federal, and CHAP referral/intake regulatory requirements.
- Establish and maintain positive working relationships with referral sources to enhance perceptions of Heart to Heart Hospice.
- Gather, collate, and report referral statistics, including key customer referral trends.
- Participate in strategic planning, identifying opportunities for service improvements.
- Maintain working knowledge of company contractual relationships and ensure admissions align with contract provisions.
- Participate in quality assessment and performance improvement activities.
- Maintain working knowledge of community resources and assist referral sources in accessing them.
- Ensure timely submission of all documentation.
- Perform other duties as required.
Requirements
- At least three to five years of experience in health care, preferably in hospice operations.
- Admissions/Intake and customer service experience.
- Valid Driver’s License and Automobile Insurance.
- Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
- Experience with EMR software.
