About the Role
The Residential Services Coordinator 1 assumes primary responsibility for developing the RSP, performing, and coordinating services and functions necessary to enhance resident autonomy, quality of life, and recovery process within the DePaul Neighborhood of the Arts (NOTA) Apartment Treatment Program.
Responsibilities
- Develops the overall Residential Service Plan with the resident and their support network.
- Provides teaching, monitoring, and recovery-oriented supports in accordance with OMH 595 and 593.
- Ensures comprehensive planning for admission, ongoing services, and discharge in a person-centered approach.
- Participates in monthly meetings with the Primary Counselor and Resident to monitor recovery efforts.
- Conducts quarterly reviews of the Residential Service Plan with the individual.
- Provides guidance and oversight to Counselors regarding resident observation, monitoring, documentation, and reporting.
- Provides guidance and oversight to Counselors for scheduling resident appointments and arranging transportation.
- Works collaboratively with the resident’s community service providers to ensure continuity of care.
- Serves as the key liaison between the resident and their clinical provider.
- Helps residents plan for purchasing clothes and other necessities.
- Works with residents to encourage healthy connections with family and community ties.
- Respects and maintains resident confidentiality and demonstrates a caring, positive attitude.
- Provides safe transporting of residents when needed.
- Follows all safety rules and regulations.
- Performs any other duties necessary for the effective operation of the program.
Requirements
- QMHS Standard Bachelor’s degree in Human Service field or an Associate’s degree in Human Service field plus three years of FT related experience or a High School Diploma plus five years of FT Human Service experience.
- Clean NYS driving record as outlined in DePaul’s personnel policy.
- Must be at least Eighteen (18) years of age.
