About the Role
Halcyon Home is seeking a Full Time Hospice LMSW to support our growing geriatric Hospice population in Austin, TX and surrounding areas. We offer competitive pay and wonderful team support.
Responsibilities
- Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
- Conduct comprehensive assessments of patients' social and emotional needs.
- Assist with coordination of care and access to community resources.
- Maintain detailed and accurate documentation of patient interactions and progress.
- Utilize Homecare Homebase EMR (knowledge of which is a plus).
- Develop personalized care plans in collaboration with the interdisciplinary team.
- Provide counseling and support to patients and families facing end-of-life challenges.
Requirements
- Licensed as an LMSW.
- Knowledge or experience utilizing Homecare Homebase EMR would be a plus.
- Minimum of one year of hospice care experience.
- Minimum of one year of social work experience.
- Compassionate and empathetic approach to working with the elderly.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time effectively.
