About the Role
Pan American Square, a DePaul Community Residence-Single Room Occupancy (CR-SRO) Program, is hiring a Community Living Staff 2 to assist residents with their service plan goals and objectives in a person-centered approach.
Responsibilities
- Provide teaching, monitoring, and recovery-oriented supports including Assertiveness/Self-Advocacy Training, Community Integration/Resource Development, Daily Living Skills, Health Services, Medication Management/Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
- Assist with supervising resident medications in accordance with physician's orders, agency policy, and NYS Office of Mental Health regulations.
- Maintain accurate and timely documentation in resident medication records.
- Communicate resident mental and physical status information to the Residential Services Coordinator, Supervisor, or provider.
- Complete basic documentation in resident files regarding psychiatric or medical concerns, progress, or other significant events.
- Ensure daily tasks and assignments are completed as directed, including preparing resident rooms, orienting new residents, and assisting with discharges and transfers.
- If working the overnight shift, complete rounds at least 4 times per shift to monitor the program and intervene in potential problems or disputes.
- Work with Overnight security to ensure resident safety and well-being, including building and grounds observation, enforcing visitor hours, securing doors, restricting unwanted visitors, and monitoring security cameras.
- Share information with supervisor or administrative staff regarding problems or concerns occurring during the shift.
- Assist residents in maintaining the upkeep of their personal rooms.
- Coordinate and participate in resident recreational activities.
- Respect and maintain resident confidentiality and demonstrate a caring, positive attitude.
- Attend staff meetings and trainings as required.
- Act as a member of the residential team, providing input for treatment team meetings and Continued Stay Reviews.
- Provide safe transporting of residents when needed.
- Perform any other duties necessary for the effective operation of the program.
Requirements
- Minimum of High School diploma or GED.
- Ability to demonstrate verbal and written skills necessary to carry out services for the success of the program.
- At least one-year of experience in a residential setting working with people in various stages of recovery from mental illness preferred.
- Valid New York State driver’s license that meets qualifications set by agency’s clean driver record policy.
- Must be at least Eighteen (18) years of age.
