About the Role
The Admissions Coordinator supports admissions to the Jewish Home & Rehab Center, acting as a liaison between the Admissions team, the community, and the Home’s Interdisciplinary teams to ensure a safe transition between care settings for new residents. This role is integral in achieving the organization’s census goals, growth, and revenue objectives.
Responsibilities
- Assist in the management of LTC census and patient/resident throughput, focusing on meeting or exceeding stated census goals and targets.
- Support the management of application throughput and data entry via various programs and modalities.
- Handle the admitting process for short-term to LTC and LTC applications before patient arrival from the hospital.
- Monitor and provide ongoing phone, email, and customer service support for Short-Term Care and Long-Term Care inquiries, including providing tours and responding promptly.
- Communicate with Case Managers regarding the admission process and updates for a smooth transition from hospital care.
- Assess and review Long-Term Care applications for completeness and appropriateness, communicating with applicants until admission is finalized.
- Collaborate with the patient financial counselor to complete financial reviews, insurance eligibility, and assist with Medi-Cal and SSI communication.
- Work with social services/TCCs and the LTC clinical team to process and assess clinicals and roommate compatibility.
- Manage the waitlist.
- Manage and communicate move-ins to LTC to avoid long-term vacancies.
- Perform room checks to ensure readiness for admission and order DME as needed.
- Work with Clinical Liaisons to identify and follow up on clinical needs/requirements for admission.
- Create and complete admissions paperwork for incoming admissions, inputting data into EMR, taking patient photographs, and uploading documents.
- Look up, review, and verify insurance benefits/eligibility, obtain authorizations, and correctly input payer information into EMR.
- Establish and maintain professional relationships with referral sources, physicians, and other healthcare professionals to increase referrals.
- Represent JHRC professionally in the community, demonstrating patience, a positive disposition, and enthusiasm.
- Attend staff meetings and educational in-services.
- Perform other duties as assigned, including cross-training and coverage for other positions within the department.
- Create admissions paperwork, including payer information and general information about the Home.
- Create name tags and information sheets for incoming admissions.
- Run necessary screenings for potential admissions.
- Complete admissions paperwork and upload documents to EMR.
- Prepare and pass Admissions Memo to respective nursing units.
- Welcome newly admitted patients, assist them to their room, and familiarize them with room amenities.
- Provide general customer service to patients/families as needed.
- Take patient photographs and upload them to EMR.
- Provide tours of the units for potential patients/families.
- Assist the team in answering call lights promptly and meeting patient needs.
- Monitor cleanliness of specific areas and report cleaning or repair requests.
Requirements
- High school diploma or equivalent required.
- Skilled nursing billing & insurance experience preferred.
- Demonstrated proficiency with MS Office, including Outlook, Word, and Excel, required.
- Demonstrated experience working in a fast-paced environment, required.
- Ability to multi-task with a sense of urgency while maintaining sensitivity, required.
- Demonstrated high level of interpersonal skills to handle sensitive and confidential situations.
- Demonstrated ability to develop and build strong, lasting relationships with referral sources, clients, colleagues, and families.
- Excellent organization, customer service, and communication skills, both written and verbal.
- Ability to lift, push, pull a minimum of twenty-five (25) pounds.
- Ability to exert maximum muscle force to lift, push, pull or carry.
- Ability to use abdominal and lower back muscles over time without fatigue.
- Ability to stand and/or sit for extended periods.
- Ability to bend, stoop, stretch, twist, sit, and reach.
