About the Role

Family Service League is seeking a part-time Case Manager for a homeless shelter in Smithtown, NY. The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, linking clients with community-based service resources, and assisting clients in achieving self-sufficiency through advocacy, counseling, and resource referrals.

Responsibilities

  • Complete new client intakes and explain shelter rules and regulations.
  • Meet with clients at least twice weekly for face-to-face meetings.
  • Develop and complete Independent Living Plans for clients.
  • Complete Mental and/or Physical Impairment forms for Independent Living Plan Violations.
  • Complete SPA applications for eligible clients.
  • Collaborate with Housing Specialists to find permanent housing options.
  • Conduct twice weekly client safety inspections and address safety issues.
  • Address unit cleanliness and client hygiene, assisting with daily living skills.
  • Collaborate with the Office Manager on client shelter obligations.
  • Work with clients to create budgets, identifying needs and obligations.
  • Review, complete follow-up, and submit FSL Client Incident Reports.
  • Maintain and update a resource database of social service organizations.
  • Provide ongoing support and counseling for clients.
  • Provide crisis intervention to shelter clients.
  • Foster inter-agency collaboration for client long-term stability.
  • Maintain up-to-date documentation in HMIS and client files.
  • Complete and submit monthly reports and documentation to the DSS Housing Liaison.
  • Attend Case Management team meetings and individual supervision.
  • Enforce shelter rules, regulations, and policies.
  • Set and observe appropriate boundaries with clients, maintaining confidentiality and HIPPA protocols.
  • Provide assessment, crisis intervention, and individual, family, and group services.
  • Advocate for client needs.
  • Provide transportation as needed.
  • Collaborate with internal and external treatment or service coordination teams.
  • Model professional behavior.
  • Attend staff meetings and required trainings.
  • Collaborate with Security to resolve client incidents.
  • Read and understand the Policy and Procedure Manual.
  • Arrive at work on time and ready to work, obtaining supervisor approval for schedule changes.
  • Read email daily.
  • Perform other duties as assigned.

Requirements

  • Bachelor’s degree in Human Services required.
  • Master’s degree in Social Work preferred.
  • Minimum of 1 year of experience in human services or related field preferred.
  • Knowledge of the Suffolk County Department of Social Services preferred.
  • Knowledge of human services and previous experience working with at-risk populations preferred.
  • Excellent interpersonal, verbal, and written communication skills required.
  • Proficient computer skills, including Microsoft Office required.
  • Experience with HMIS, Google Apps, and web-based project management software preferred.
  • Bilingual in Spanish preferred.
  • Valid and clean NYS Driver's License required.
  • Ability to sit and stand for extended periods.
  • Ability to perform moderate lifting.
  • Ability to go up and down stairs.
