About the Role

Ensure the development and implementation of an individualized, trauma-informed, and holistic treatment plan for children, maximizing participation of the child and key family members. Provide clinical services specific to the child and family’s needs to accomplish identified discharge/reunification outcomes while working collaboratively through initial and ongoing assessments.

Responsibilities

Provide initial and ongoing assessments of children and their families.
Act as the lead clinical member of the inter-disciplinary treatment team for each child.
Develop and implement comprehensive, individualized, and discharge/reunification-focused treatment plans using a strength-based approach.
Monitor and revise treatment plans as necessary.
Provide individual, family, and/or group therapy for children and families.
Ensure clinical contact frequency, nature, and location comply with agency policy and regulatory requirements.
Support Case Planner activities to ensure full integration of all services provided to each child.
Keep referring/custodial agencies and parents informed of case movement, planning decisions, and significant events.
Monitor child's daily functioning to ensure continuity of service in accordance with the treatment plan.
Coordinate treatment efforts with other team members via meetings and collateral contacts.
Facilitate timely client transfers within the agency and discharge planning to outside services.
Facilitate collateral contacts with external individuals, schools, and agencies relevant to the treatment plan.
Be prepared to testify in court related to clients’ clinical services.
Participate in additional meetings as deemed necessary.
Maintain required documentation accurately and in a timely manner via the agency’s Electronic Health Record (EHR) and as required by regulatory bodies.
Complete psychosocial assessment and/or semi-annual assessment updates.
Develop individualized, comprehensive treatment plans with specific discharge criteria, client-centered goals, measurable objectives, child/family participation, and modalities/methods for discharge.
Provide periodic progress notes relevant to treatment plan goals and objectives.
Document treatment progress as required by agency standards and regulatory bodies.
Complete other documentation associated with service provision (e.g., incident reports, court reports).
Partner with staff from other disciplines and/or Foster Parents for ongoing program evaluation and development.
Provide staff training in treatment-related skill areas.
Provide crisis management and problem-solving support services for caseload.
Participate in program-specific on-call procedures when assigned for child-specific clinical issues.
Successfully complete all defined training.
Provide training as requested by supervisor and the agency Training Department.
Maintain and improve clinical skills and competency through active participation in supervision and training programs.
Attend professional workshops or conferences as defined by program and agency competency standards.
Actively participate in performance improvement activities.
Perform other associated tasks as assigned by supervisor.

Requirements

Master’s degree in social work, mental health counseling or marriage and family therapy.
Licensure or limited permit required.
One-year post-graduate experience in a child welfare or behavioral health setting highly preferred.

Ability to communicate with parents regarding developmentally appropriate behavior and expectations for children.
Supports the parenting role by providing relevant information about child development and learning.
Current knowledge of appropriate holistic interventions for children and families and the impact of trauma on development.
Demonstrated ability to form significant clinical relationships with children and their families.
Demonstrated proficiencies in the provisions of individual, family and group therapies.
Strong written, verbal and organizational skills.
Demonstrates understanding of 29-I, and other health related and core services as needed.
Demonstrated proficiency in goal and objective writing.
Familiarity with regulations relevant to clinical services, clinical documentation and confidentiality.
Sensitivity and awareness of issues related to cultural diversity.
Demonstrated knowledge of documentation requirements of licensing and accrediting bodies.
Ability to engage in effective team building and collaborative working relationships.
Works closely with Case Planners to facilitate holistic treatment and effective Case Management.
Maintains positive relationships.
Valid New York State driver license.
