About the Role

Provide 1:1 adult neurorehabilitation in patients’ homes and community settings. Serve as the primary treating SLP from evaluation through discharge. Design a flexible work schedule that fits your lifestyle, seeking approximately 15 hours per week across 3 days.

Responsibilities

Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders.
Re-assess treatment plan effectiveness, attend staffing meetings, and participate in family conferences.
Conduct discharge evaluations, recommend home and community safety measures, and provide referral and educational resources.
Communicate patient needs and progress to the treatment team, physician, patient, and family members.
Collaborate with other disciplines to ensure coordinated care and enhance patient outcomes.

Requirements

Minimum of a Master’s Degree from an accredited Speech Language Pathology program.
Valid Speech Language Pathology license in the state(s) of practice.
Current CPR Certification in accordance with state regulations.
A minimum of one year’s work experience as a Speech Language Pathologist.
Demonstrates knowledge of rehabilitation techniques related to complex neurological injury preferred.
Effective and professional verbal and written communication skills.
Ability to lift 50 pounds.
Ability to move, lift, or transfer patients, potentially up to 100 pounds, following safety procedures.
Duties require fine motor skills, visual acuity, and walking/standing for extended periods.
Additional physical requirements include frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
A health screen or examination may be required to verify physical capability for assigned duties with or without reasonable accommodations.
