About the Role
Under the direction of the Residence Manager, provides care, rehabilitative training, support and supervision for the development and/or enhancement of community living skills to residents at the DePaul Rutherford Place OMH Licensed Congregate Treatment Site. Evening and Overnight shifts will receive a $1 shift differential.
Responsibilities
- Provide teaching, monitoring and recovery oriented supports in accordance with OMH 593 and 595 including Assertiveness/Self-Advocacy Training, Community Integration/Resource Development, Daily Living Skills, Health Services, Medication Management Training, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
- Ensure comprehensive planning in a person-centered approach for resident admission, ongoing recovery-oriented services, and discharge.
- Observe, monitor, document, and report resident behaviors and progress according to agency standards.
- Assist residents in preparing nutritious meals.
- Assist in planning, implementation, and support of recreational activities.
- Provide safe transportation for residents when needed.
- Complete assigned household chores to maintain cleanliness and safety standards.
- Supervise and document resident medications per agency policies.
- Perform other duties necessary for the effective operation of the community residence.
Requirements
- RC I: High school diploma or GED required.
- RC I: Human services experience preferred.
- RC I: Driving record that meets agency’s “clean driving record policy”.
- RC I: Must be at least Eighteen (18) years of age.
- RC II: QMHS Standard.
- RC II: Bachelor’s degree in Human Services, or an Associate’s degree in Human Services with 3-years FT related paid experience, or a High school diploma and 5 years related paid experience.
- RC II: Driving record that meets agency’s Clean Driving Record Policy.
- RC II: Must be at least Eighteen (18) years of age.
