About the Role
Brookdale at Home is seeking Certified Nursing Assistants (CNAs) for one-on-one assignments helping seniors. This role offers flexible scheduling with full-time, part-time, and PRN positions available. Responsibilities
- Provides direct care to clients in their homes, promoting their highest level of functioning.
- Assists in maintaining a safe and neat client environment.
- Promotes quality services within non-skilled home care standards and state regulations.
- Works independently in the client's home with supervision and support as per state regulations.
- Communicates changes in client care needs and service requests to the Agency Leader.
- Provides personal care, concierge, homemaker, and companion services per the individualized plan of care.
- Documents all pertinent information regarding care as assigned.
- Assists clients with activities of daily living, including bathing, dressing, grooming, toileting, transferring, and meal assistance.
- Prepares clients for meals and assists with feeding.
- Assists clients with toilet and incontinence care.
- Keeps client's home neat and clean.
- Performs vital signs and other trained procedures as per client's care plan.
- Completes prevention and restorative care according to the plan of care.
- Maintains a working knowledge of the home care agency’s policy & procedures.
- Assists in turning, positioning, and repositioning of clients.
- Communicates effectively and proactively with clients, families, visitors, and the Agency Leader and scheduler.
- Actively participates in the Community Quality Assurance Program.
- Maintains education development to stay compliant with State and Federal Regulations and current client care practices.
- Maintains client confidentiality.
- Assists clients with homemaker companion tasks such as laundry, shopping, light housekeeping, and meals.
- Follows infection control and safety policies and guidelines.
- Maintains means to travel to client’s homes to arrive on time for assignment.
Requirements
- High school diploma or General Education Degree (GED) is required.
- Proof of completing an approved Certified Nursing Assistant program is required.
- Six months of experience working independently with clients in their own homes providing healthcare/homecare services is preferred.
- Successful completion of State C.N.A. course.
- Must maintain CNA certification.
- Reliable transportation is required.
- Must have a valid state identification card.
- Ability to lift up to 50 pounds.
- Requires interaction with co-workers, residents or vendors.
- Occasional weekend, evening or night work if needed to ensure shift coverage.
- Possible exposure to communicable diseases and infections.
- Potential injury from transferring, repositioning, or lifting residents.
- Exposure to latex.
- Possible exposure to blood-borne pathogens.
- Possible exposure to various drugs, chemical, infections, or biological hazards.
- Subject to injury from falls, burns, odors, or cuts from equipment.
- Has a basic level of skill and can follow written or verbal instructions.
- Has basic technical skills to complete repetitive and well-defined duties.
- Has a basic knowledge of the organization to answer basic questions.
- Must have working knowledge of facility emergency procedures including fire and disaster drills.
- Ability to follow facility policies.
- Ability to maintain resident confidentiality and adhere to HIPAA policies.
