About the Role
The Community Rehabilitation Specialist helps adults, children, and youth build the skills and supports they need to live, learn, work, and participate in the community. This position focuses on helping individuals identify employment and life goals, connect with supports, strengthen daily living and coping skills, and maintain progress toward recovery and greater independence. The role works closely with the treatment team, community partners, and the individual served to support person-centered care and successful community and employment integration.
Responsibilities
- Support and assist the Director and designees with relevant program responsibilities.
- Participate as a member of multidisciplinary teams to support increased patient engagement and adherence to treatment.
- Convey information inter/intra agency and communicate professionally with the team regarding changes and conflicts.
- Develop relationships with community stakeholders to enhance awareness, adopt harm reduction strategies, reduce stigma, and facilitate client care.
- Use standardized assessment tools, engagement process, person-centered planning, and harm reduction methods to offer vocational rehabilitation services.
- Provide rehabilitation support and recovery-oriented services tailored to individuals coping with substance use, psychiatric or emotional challenges, and/or co-occurring mental health and substance use.
- Facilitate skill development and therapeutic interventions that promote self-management and reduce the risk of recurrence of mental health or substance use episodes.
- Help clients improve daily living skills essential for successful functioning in home, school, work, and community settings.
- Design and implement recovery plans aimed at enhancing independence and quality of life.
- Help individuals identify meaningful life goals and related objectives through a person-centered assessment process.
- Monitor progress and adjust support strategies based on individual needs and goals.
- Document progress in the electronic health record and track and report all requested data.
- Maintain all laws and policies concerning corporate compliance, confidentiality, security, and client rights.
- Exercise problem-solving and conflict resolution.
- Perform other duties as assigned or that may develop.
