About the Role
This grant-funded position is integral to the multidisciplinary healthcare team, focusing on mental health and substance abuse interventions, psychosocial assessments, and addressing social determinants of health. The role advocates for patients' social work and behavioral health needs within a team-based care model and may require travel between sites.
Responsibilities
- Apply empowerment-based prevention and intervention strategies to improve community health.
- Offer the “person-in-environment” perspective, incorporating all factors influencing a patient’s health care experience.
- Assess individual needs, strengths, barriers, and readiness to change.
- Address personal and environmental issues to maximize patient wellbeing and appropriate use of healthcare resources.
- Utilize the DSM-5 for diagnostic impressions of mental health and/or substance use disorders.
- Develop and implement treatment plans.
- Conduct direct delivery of mental health services and behavior change interventions, including therapy and brief interventions.
- Conduct crisis counseling as needed.
- Initiate appropriate referrals to outside mental health/substance abuse treatment or community resources.
- Help patients and families understand illness, treatment options, and consequences of treatment decisions.
- Provide expertise, consultation, and education on psychosocial issues affecting patient care.
- Communicate and collaborate with the care team to assess patient needs and provide comprehensive, patient-centered healthcare.
- Complete all direct and indirect care documentation in a timely manner, ensuring patient records are current and complete.
- Attend daily patient care huddles, clinic huddles, meetings, patient conferences, and planning sessions.
- Attend seminars and maintain licensure and/or certification requirements for continuing education.
- Participate in quality strategies to evaluate compliance with evidence-based guidelines and identify opportunities for improvement.
- Ensure all patient care tasks comply with regulatory and accreditation standards, including The Joint Commission and CommUnityCare SOPs and policies.
- Develop and maintain favorable internal relationships with co-workers, managers, staff, and providers.
- Interact respectfully and collaboratively with patients and their families.
- Seek opportunities and implement goals for professional development.
- Perform other duties as assigned.
Requirements
- Master’s degree in Social Work (MSW).
- Minimum of 2 years’ experience in a field related to counseling individuals in crisis/trauma situations.
- Current Clinical Social Worker License (LCSW) in Texas.
- Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross.

- Bilingual (Spanish/English) preferred.
