About the Role
ADAPT Community Network provides services to people with developmental disabilities. This role involves developing and implementing Individualized Service Plans, providing case management, and coordinating services for individuals in a residential setting.
Responsibilities
- Provides social services to maximize social functioning, enhance family coping capacity, safeguard human and civil rights, and foster human dignity.
- Develops and monitors individual treatment plans in accordance with regulatory and agency policy.
- Participates in interdisciplinary planning, interprets, documents, and follows up on recommendations.
- Chairs interdisciplinary team meetings.
- Establishes communication channels with residential and professional staff to identify needs as individuals progress through habilitation.
- Develops goals and activities generated by the interdisciplinary team.
- Produces an active treatment plan schedule for each person served.
- Documents comprehensive functional assessments and reviews of the plan of care.
- Ensures proper data collection through observation and in-service training.
- Assists in all activities of daily living and participates in off-site activities.
- Supervises Residence Program Specialists in the implementation of active treatment plans.
- Makes regular and meaningful family, residential, and agency-wide contacts.
- Trains and instructs Residence Program Specialists on specific goal plans, data collection, daily routines, and general procedures.
- Maintains contacts with government and not-for-profit social service agencies to access resources and advocate for services.
- Performs individual evaluations to determine the need for alternate placement.
- Handles family outreach and in-service training.
- Ensures timely application for all eligible benefits and transfer of prior accumulated funds.
- Provides systematic follow-up to assure referral to appropriate community agencies upon discharge.
- Maintains security of confidential information, exercising discretion.
- Coordinates admissions to the program, including preparation of referral material, initial screening, and contact with families and referral services.
- Ensures all required adaptive equipment is secured and maintained.
- Maintains up-to-date files on applicants.
- Processes new admissions, including family contact and arrangements for day programming.
- Advocates for the rights of the people supported.
- Writes monthly case notes on interpersonal, social, workshop progress, and family contacts.
- Serves as a liaison to Day Programs, Service Coordination, and Clinics.
- Keeps abreast of changes in the field.
- Performs other related duties as requested.
Requirements
- Ability to read, write, speak, and understand English.
- Good interpersonal skills for effective interaction with employees, co-workers, individuals supported, families, and outside contacts.
- Excellent organizational skills.
- Proficiency in personal computer use and relevant software.
- Ability to maintain strict confidentiality.
- Ability to work independently and handle multiple tasks.
- Ability to work flexible hours.
