About the Role
The Clinical Care Program is seeking a Social Worker 2 for the Downtown East Clinic Outpatient team, working with adults with severe and persistent mental illness. The program is client-centered, with treatment based on recovery principles for individuals with major mental illness, predominantly Schizophrenia and Concurrent Disorders.
Responsibilities
- Participate in the assessment and treatment process to engage clients in their recovery goals.
- Conduct psychosocial-based assessments.
- Provide financial and social supports/assistance and counseling to clients, family, and caregivers.
- Address the social determinants of health through client psychosocial activities.
- Formulate treatment plans.
- Complete standardized assessments.
- Utilize motivational interviewing techniques.
- Implement relapse prevention strategies.
- Provide Concurrent Disorders Case-management.
- Facilitate Concurrent Disorders group programming.
- Monitor client response to therapeutic interventions.
- Communicate with client's social and community networks.
- Network with other referral sources to coordinate client flow.
- Liaise with community providers.
- Participate in the education of clients, families, and mental health providers.
- Provide supervision of Masters of Social Work students.
- Collaborate with faculties of social work on special projects.
- Contribute to the development of best practices in the social work field.
- Teach other professionals.
- Support a workplace that embraces diversity, encourages teamwork, and complies with regulatory requirements.
Requirements
- Master’s of Social Work degree.
- Registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers.
- Eligible for an adjunct faculty appointment with affiliated faculties of social work.
- Knowledge of relevant community resources and working with families/significant others.
- Demonstrated knowledge of mental health and substance use issues.
- Excellent organizational, interpersonal, and written communication skills.
- Ability to work effectively in a fast-paced clinical environment.
- Ability to work effectively as a team member in a dynamic inter-professional environment delivering the Flexible Assertive Community Treatment Model (FACT).
- Demonstrated knowledge and experience in assessment and rehabilitation.
- Fundamental knowledge and application of the recovery model and current Best Practices for Complex Care and Recovery Program clients.
- Experience working with clients, staff, and organizations of diverse ethno-racial backgrounds.
- Computer skills, including word processing, electronic communication, and workload measures.
