About the Role
Change Grow Live is a charity dedicated to making a difference in the lives of Service Users by offering support and respect in a safe environment. The Housing Support Recovery Coordinator will assess and engage with service users, ensuring they access the right intervention at the right time. This role involves supporting users to navigate treatment, working in partnership with Housing Officers to address substance misuse and tenancy sustainment. The post-holder will be part of a team providing screening, assessments, and care planning for new service users, delivering harm minimization training, and coordinating treatment for clients with housing needs.
Responsibilities
- Establish effective communication and referral pathways for individuals at risk of homelessness and in contact with other health and care agencies.
- Provide wrap-around support in emergency, temporary, and supported accommodations.
- Prioritize caseload based on crisis intervention and severity of needs.
- Attend multi-disciplinary team meetings to support links with health and social care services, peer support networks, community assets, and mutual aid.
- Work closely with the local authority, Health & Social care sector, partners, and provisions linked to preventing homelessness to improve joint working.
- Enhance partnership working with primary care, mental health services, housing, and other voluntary organizations supporting individuals at risk of losing tenancies.
- Provide wrap-around support on budgeting, education, training, and employment while supporting engagement in structured services.
Requirements
- An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment, and personal relationships.
- Minimum of 12 months of experience facilitating successful tenancy sustainment and working towards positive community connections.
- Experience engaging with a wide variety of individuals with diverse backgrounds and needs.
- Ability to work with and manage challenging behavior.
- Must have the right to work in the UK.
- This role is not eligible for visa sponsorship.
