About the Role
Premier Health Partners, P.C., a partner of Erie County Medical Center (ECMC), seeks a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality patient care and assist with medical procedures. This role is crucial for advancing wellness through trusted relationships, innovation, and patient-centered care.
Responsibilities
- Prepare patients for examination by performing preliminary physical tests, including manual blood pressure, height, weight, BMI, and pulse.
- Assist with patient assessments and document medical histories.
- Maintain patient care equipment.
- Observe and document patient condition according to nursing standards.
- Ensure a safe, secure, and healthy work environment by following internal and legal regulations.
- Assist providers during examinations and treatments by handing them instruments and materials.
- Administer immunizations and injections; may perform EKG testing, pulse oximetry, and phlebotomy.
- Transport patients in various healthcare settings.
- Teach patients and families about health conditions, treatment plans, and preventive care as outlined in the care plan.
- Inform nurses or managers of any changes in patient status.
- Ensure accurate documentation of patient care in Electronic Medical Record (EMR) systems.
- Deliver exemplary customer service to ensure a positive organizational experience.
- Perform other duties assigned by management.
Requirements
- Possession of a license and current registration to practice as a Licensed Practical Nurse by the New York State Education Department.
- 1-2 years of nursing experience in an ambulatory care setting is preferred.
Skills
- Strong clinical skills, including taking vital signs, administering medication, updating patient charts, and assisting with minor procedures.
- Proficiency in Electronic Medical Records (EMR) and other healthcare technology.
- Ability to perform patient assessments, develop care plans, and manage follow-up care.
