About the Role

As the Case Manager, you will play a pivotal role in our organization, assisting families in providing them with the necessary care and resources needed.

Responsibilities

  • Maintains and updates digital and physical records of all families as required by governing agencies.
  • Collaborate with clients to develop service plans based on an assessment of each family’s needs.
  • Assist families with referrals to outside agencies for employment, education, parenting, legal, medical, mental health services, and any other services the family may need.
  • Maintain and update case files bi-weekly to document progress.
  • Assist families with moving into permanent housing.
  • Ensure that clients maintain an open public assistance case.
  • Adhere to all city and state guidelines for maintaining documents.
  • Complete all assigned staff trainings.
  • Conducts unit inspections.

Requirements

  • Familiarity with case management.
  • Knowledge of Outlook, Word, and Excel.
  • Bilingual in Spanish or other language is a plus.
