About the Role

Responsibilities
- Schedule medical appointments for individuals and arrange transportation.
- Ensure medications are ordered, documented, and provided as directed.
- Perform regular checks on medications and medical supplies.
- File and maintain medical records in accordance with agency policies.
- Document all activities in the electronic medical record system.
- Provide direct care as needed, ensuring individuals’ needs are met.
- Escort individuals to and from medical appointments.
- Prepare individuals for clinician visits.
- Report pertinent clinical findings to the nurse or clinician.
- Regularly check and replenish medical supplies.
Requirements
- Previous experience as a Direct Support Professional (DSP), Nursing Assistant, or Patient Care Assistant is a plus.
- Strong organizational skills, ability to multitask, and ability to work under pressure.
- Must have a current and clean New York State Driver’s License.
- Must be able to drive a 15 passenger van.
- Knowledge of OPWDD policies and state/federal regulations.
- Must be fully vaccinated (COVID-19).
