About the Role
The Activation Worker plans and conducts daily activation, recreational, and social programs for residents of the LTC Home, aiming to improve and maintain their quality of life. This role is crucial in shaping the culture, resident experience, and care environment of the new Ng Siu Chan Centre, which is built on principles of Relationship-Centred Care and Meaningful Living.
Responsibilities
- Conducts timely and accurate assessment of residents upon admission and on a quarterly basis, preparing and monitoring the effectiveness of care plans.
- Plans and implements innovative regular and seasonal recreational and social activities to improve residents' quality of life, considering their feedback, needs, and preferences.
- Ensures activities are conducted in a safe, hygienic, and supportive environment, including one-on-one, small group, and large group activities.
- Assists the Activation Manager in supervising volunteers who assist or facilitate activation programming.
- Mentors and works effectively with students and interdisciplinary team members.
- Maintains and updates resident records.
- Works with staff of other disciplines in the LTC Home to provide quality care to residents.
- Shops for cost-effective and appropriate program supplies and maintains accurate records of fees and expenditures.
- Assists the Activation Manager in providing information and education to residents and their family members.
- Participates in in-service programs and professional development.
- Keeps up-to-date with developments and best practices in the field.
- Performs other duties as assigned.
Requirements
- Diploma in Recreation and Leisure Studies, Therapeutic Recreation, Kinesiology, or equivalent.
- Experience in a social services or long-term care setting is an asset.
- Experience and skills in facilitating activation and therapeutic recreation programs, preferably working with the geriatric population.
- Creativity and innovativeness in developing meaningful programs for clients within a tight budget.
- Superb customer service skills.
- Demonstrated initiative to support clients and the team in a positive and efficient manner.
- Computer literacy.
- Knowledge of Chinese word processing is an asset.
- Current CPR Certificate.
- Current Food Handler Safety Certificate.
- Satisfactory Police Vulnerable Sector Screen.
- Ability to communicate in English and Chinese.
- Some lifting and transfer.
- Occasional travel within the Greater Toronto Area.
- Use of own vehicle.
- Successful applicants must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.
