About the Role
Provide quality incident investigations for all allegations within the agency, ensuring appropriate recommendations are made to safeguard individuals and provide the highest quality of services. This role also involves educating and training staff in abuse prevention, incident reporting, and positive approaches, while ensuring compliance with all applicable standards.
Responsibilities
- Maintain current knowledge of incident reporting regulations (14NYCRR Part 624 and Part 625) and NYS Justice Center/OPWDD directives.
- Complete OPWDD 147 and initial IRMA entry for reportable incidents, collecting statements and documentation for preliminary investigations.
- Conduct thorough and quality investigations, meeting regulatory and agency requirements.
- Monitor incident follow-up in electronic records, ensuring adequate documentation of corrective actions by the due date.
- Upload assigned incident investigations into IRMA.
- Maintain investigation electronic folders.
- Meet with supervisor to review investigation status, follow-up, and performance.
- Inform supervisor of significant investigation concerns.
- Complete Notable Event reviews and ensure all follow-up and corrective actions are addressed.
- Provide training to employees on Incident Reporting and Abuse Recognition.
- Perform other duties relevant to the position as requested.
- Advise supervisor of any obstacles to achieving responsibilities.
Requirements
- Associates Degree, Bachelor’s Degree preferred.
- Knowledge of OPWDD Part 624 Regulations, preferred.
- Previous experience in Quality Improvement/Compliance within OPWDD, OMH, DOH, OASAS, or OCFS preferred.
- Successful completion of OPWDD/NYS Justice Center for the Protection of Vulnerable Persons Incident Investigation Training, upon hire.
- Excellent verbal, listening and writing skills.
- Strong typing skills and experience using Word.
- Ability to work independently and motivate others.
- Must be at least 18 years of age.
- Reliable transportation (NYS Drivers’ Licensed required).
- Maintain all required certifications/training by State regulations and CDS Monarch policy.
- Adhere to all CDS Life Transitions and Monarch policies and procedures.
- Attend mandatory Education and Training modules as scheduled.
- Obtain and maintain required certifications.
- Act as a professional representative of CDS Life Transitions/CDS Monarch.
