About the Role
Nova Scotia Health seeks a Family Physician with Certificates of Added Competencies (CACs) in Care of the Elderly (COE) to enhance care for older adults in the Cape Breton Regional Municipality. The role involves direct patient care, consultations, peer support, and education, acting as a community system leader.
Responsibilities
- Provide advanced-level patient care and consultation for issues affecting older adults' health, physical and cognitive function, and independence.
- Support peer-to-peer education and consulting to enhance continuity and capacity for comprehensive care.
- Lead or participate in interprofessional healthcare teams, particularly in Long Term Care, using an advance Care by Design approach.
- Provide leadership and education on healthcare for older adults to various learners.
- Work in hospital settings as a consultant or the most responsible physician in areas like rehabilitation, acute care, post-acute facilities, and emergency rooms.
- Collaborate with service providers (EHS, CC) and government authorities for initiatives like Hospital at Home.
- Engage in research to advance the field of healthcare for older adults.
Requirements
- Eligible for Licensure with the College of Physicians and Surgeons of Nova Scotia (CPSNS).
- Eligible for certification with the College of Family Physicians of Canada (CFPC).
- Certificate of Added Competencies in Care of the Elderly (COE) required.
- Evidence of insurance with the Canadian Medical Protective Association (CMPA).
- Must be able to commute independently to various care settings including hospital, home, long-term care, and assisted living residences.
