About the Role
Our Bereavement Coordinator steps in to assess needs, counsel, and facilitate support for grieving family members, providing compassionate care for emotional, social, and spiritual needs.
Responsibilities
- Collaborate with other departments within the hospital, to align interdepartmental functioning, goals, and expectations as outlined in the SLA.
- Conduct comprehensive bereavement assessments, to identify bereavement strengths and needs of patient and family.
- Conduct follow-up with grieving families to ensure their support needs are met and to gain insight into their satisfaction of care.
- Conduct preparatory meetings with the patient and family, and maintain availability during the bereavement period as needed.
- Periodically plan and organize memorial services for Hospice families.
- Provide support and counseling consistent with patient and family beliefs.
- Review and develop protocols for treatment and propose current options for interventions.
Requirements
- Master's Degree- Social Services or Divinity- Required
- 2 years- Social Services Experience- Required OR
- 2 Years- Chaplaincy Experience- Required OR
- 2 Years- Counseling Experience- Required
