About the Role

Whitney Young Health (WYH) is dedicated to providing high-quality, affordable, and accessible healthcare to a diverse community. The Social Health Engagement Specialist will conduct screenings, assessments, and navigation activities to connect patients with appropriate resources, improve health outcomes, and enhance health literacy.

Responsibilities

Conduct Health Related Social Needs (HRSN) screenings, eligibility assessments, and navigation activities.
Provide health advocacy, education, and navigation to improve health outcomes and literacy.
Facilitate access to respectful, high-quality, equitable health care.
Connect individuals to community-based services.
Provide culturally competent information to improve health and reduce barriers to services.
Maintain timely and accurate documentation of communications, referrals, and other paperwork.
Utilize ePACES/MEVS and other data sources to confirm Medicaid enrollment and HRSN screening status.
Manage member/patient consent and attestation.
Conduct HRSN screening using the AHC screening tool.
Determine eligibility for Enhanced HRSN services and refer members to eligible programs.
Use various technology platforms to ensure successful referral connections.
Maintain effective communication with internal team members, community members, and partner organizations.
Document progress notes and actions taken with each referral.
Demonstrate time management and multi-tasking skills.
Fulfill CDC-endorsed Community Health Worker core consensus competencies training requirements.
Demonstrate excellence in customer service.
Communicate HIPAA compliance, corporate compliance, and client confidentiality.
Ensure compliance with local, state, and federal regulations and accreditation standards.
Adhere to National Patient Safety Goals.
Complete other duties as assigned.

Requirements

Associate degree in Human Services or a related field.
One to three years of experience working and engaging community partners.
Strong organizational skills.
Knowledge of Excel and data tracking.
Valid NYS driver's license.
Ability to travel between sites as needed.
Minimum of 5+ years related experience in a clinical, non-profit, or managed-care organization environment (preferred).
Extensive knowledge and understanding of health equity, social drivers of health, and social care data (preferred).
Experience working or living in the target community and knowledge of community resources (preferred).
