About the Role

The role involves supporting individuals with disabilities to enhance their quality of life and satisfaction through personalized care and assistance. This includes structuring daily routines, managing living environments, and facilitating personal development.

Responsibilities

Maintain and promote the quality of life and satisfaction of residents through individual support and care.
Structure daily activities and ensure a consistent daily routine.
Design the living environment considering individual needs.
Shape the living environment to support the normalization of living conditions.
Independently prepare, implement, review, and update support, development, and care plans, involving residents.
Perform necessary household and care tasks, involving and guiding individuals with disabilities.
Assist in relevant medical procedures as per doctor's orders, including medication management, medical and nursing care, and support during medical consultations or visits to practices and hospitals.
Coordinate between contact persons and healthcare facilities, especially during somatic or psychological crises, and support/debrief crisis situations.
Develop and implement individual support plans and evaluate their effectiveness.
Support/assist in crisis management.
Maintain PC-based support and care documentation.

Requirements

Completed training as a Heilerziehungspfleger, Heilpädagoge, Altenpfleger, Kinderkrankenpfleger, Gesundheits- und Krankenpfleger, or Pflegefachmann (m/f/d), or an equivalent qualification according to § 4 SQV.
Knowledge and experience in the field of participation and care for people with disabilities.
Ability to interact empathetically and sensitively with people with disabilities.
Willingness and ability to work in a team, work in shifts, and participate in further training/continuing education/supervision.
Provide an extended police clearance certificate and a valid health certificate.
Applicable PC skills.
Driving license Class 3/B.
Ability to drive minibuses.
