About the Role

The Care Coordination Program Assistant for Prime Care Coordination (PCC) monitors and tracks care coordination administrative duties and documentation for an assigned region. This role supports the Assistant Regional Director and/or Director by uploading documents to the Electronic Health Records (EHR) system, maintaining office supplies, managing mail, scheduling meeting rooms, and reporting maintenance needs.

Responsibilities

  • Upload all required documents including enrollment, disenrollment, and annual review paperwork into PCC’s EMR.
  • Serve as the main contact for the region, communicating CAS assessment information to Care Managers and monitoring completion status.
  • Schedule meetings and coordinate the use of conference and/or meeting areas.
  • Maintain the regional Care Coordination staff roster and contact information.
  • Ensure completion and filing of pertinent documents for internal and external audits.
  • Maintain regional fax folders and distribute documents.
  • Create new enrollment folders and mail them as needed.
  • Complete non-billable notes to document completed tasks.
  • Develop and ensure consistency for regional correspondence, including creating, copying, and distributing letters, memos, reports, and forms.
  • Maintain the highest level of confidentiality, following all HIPAA policy and procedures.
  • Meet professional obligations through efficient work habits, including meeting deadlines, honoring schedules, and coordinating resources and meetings effectively.
  • Perform all other duties relevant to the position as requested.

Requirements

  • Proficient in all Microsoft applications (Excel, Word, PowerPoint, Microsoft Outlook, and Access).
  • Ability to work independently with minimal supervision.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to be efficient and productive in a fast-paced environment.
  • Must be detail-oriented and able to multi-task.
  • Adhere to all Prime Care Coordination policies and procedures.
  • Adhere to the Agency Mission, Vision, Shared Values, and Customer Service Standards.
  • Attend mandatory education and training modules as scheduled.
  • Obtain and maintain required certifications.
  • Maintain all required certifications/training by State regulations and PCC policy.
  • Act as a professional representative of PCC in regard to appearance, behavior, temperament, communication, language, and dress.
