About the Role
Edgerton Square, a DePaul Community Residence-Single Room Occupancy (CR-SRO) Program, is seeking a mental health professional to provide direct support, advocacy, and education to residents. The role involves assisting residents in developing individualized service plans and coordinating services to enhance their autonomy, quality of life, and recovery process.
Responsibilities
- Provide tours and lunch visits to prospective residents and offer input on their acceptance into the program.
- Ensure comprehensive, person-centered service planning for each resident, including recovery-oriented services and discharge planning, in compliance with NYS Office of Mental Health (OMH) standards.
- Conduct quarterly service plan reviews with residents and their support teams to assess progress and make necessary revisions.
- Provide teaching, monitoring, and recovery-oriented supports, including Assertiveness/Self-Advocacy Training, Community Integration, Daily Living Skills Training, Health Services Education, Medication Management, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.
- Observe, monitor, document, and report resident behaviors and progress according to agency standards.
- Maintain accurate and timely resident files, including progress notes, service plans, psychiatric and medical records.
- Schedule annual medical appointments (physical, eye, OB/GYN, dental) and arrange transportation as needed.
- Collaborate with Resident Health Home Care Managers and community service providers to ensure continuity of care.
- Act as a liaison between residents and their clinical providers regarding mental health monitoring.
- Provide direct assistance with daily living skills, including room upkeep, meal preparation, and laundry.
- Assist residents in planning for necessary purchases like clothing and other essentials.
- Encourage healthy connections with family and communities, and help residents develop new supports and interests.
- Coordinate and participate in resident recreational activities.
- Assist Medication Coordinators with medication supervision and assume dining room responsibilities during breakfast when assigned.
- Provide safe transportation for residents when necessary.
- Maintain resident confidentiality and demonstrate a positive attitude towards residents, staff, family, and service providers.
- Adhere to all safety rules and regulations.
- Attend required staff meetings and trainings.
- Contribute relevant input to treatment/support team meetings and continued stay reviews as a member of the residential team.
- Perform other duties as required for program operation and resident well-being.
Requirements
- Must meet one of the following education and experience criteria:
- A. Bachelor's degree in a Human Services field plus at least one year of experience in a Human Services field.
- B. Associate's degree in a Human Services field plus at least three years of related experience in a Human Services field.
- C. High School diploma/GED plus at least five years of experience in a Human Services field.
- Must possess a clean/valid New York State driving record.
- Must be at least eighteen years of age.
