About the Role
Change Grow Live is seeking an experienced Non-Medical Prescriber (NMP) to join their Barking and Dagenham service. This role involves delivering high-quality, person-centred care to service users with complex health and substance use challenges.
Responsibilities
- Assess, diagnose, plan, implement, and evaluate treatment, interventions, and care for service users.
- Prescribe and review medication for therapeutic effectiveness, adhering to evidence-based practice and national/local protocols.
- Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations, seeking guidance from the Clinical Lead when necessary.
- Assess, update, and refer service users for BBV vaccination and immunization programs.
- Support service users in adopting health promotion strategies for healthy lifestyles.
- Communicate effectively with service users and carers, anticipating and overcoming communication barriers.
- Maintain effective communication within the organization and with external stakeholders, advocating for service users and colleagues.
- Produce accurate, contemporaneous, and complete consultation records.
- Assess the effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation.
- Participate actively in the annual appraisal system.
Requirements
- Qualified Non-Medical Prescriber.
- Must be able to work the late shift on Tuesdays (11:30 am to 7:30 pm) and Saturdays.
- At least one year of relevant and recent experience working within substance misuse treatment.
- Sound and demonstrable knowledge of harm reduction and health promotion interventions.
- Ability to assess and recovery plan service users and formulate written reports.
- Ability to communicate confidently and effectively, verbally and in writing.
- Full driving licence and access to own transport, or ability to demonstrate easy movement between services.
