About the Role
Jordan Health is seeking a Community Health Worker Supervisor to provide oversight, training, and guidance to Community Health Workers, ultimately improving patient care and quality of life.
Responsibilities
- Improve performance and effectiveness of Community Health Workers with patients.
- Establish and maintain successful collaborative relationships between the Community Health Workers team and other departments.
- Establish and maintain successful partnerships with external agencies.
- Perform all duties of a Community Health Worker with own small caseload.
- Complete bi-weekly timecards for all Community Health Workers.
Requirements
- Must have a valid driver's license and reliable transportation.
- BLS through the American Heart Association.
- NYS Infection Control certification.
