About the Role
Venture Forthe is seeking a Home Infusion Therapy Registered Nurse (RN) to provide high-quality infusion therapy services to patients in their homes. This role involves clinical care, patient education, care coordination, and collaboration in a supportive environment.
Responsibilities
- Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
- Assess patients and develop individualized infusion plans of care.
- Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
- Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
- Monitor patients for infusion reactions, side effects, and clinical changes.
- Maintain accurate and timely clinical documentation.
- Support quality improvement initiatives and ongoing competency programs.
- Assist with training and supervision of paraprofessional and nursing staff.
Requirements
- Current New York State Registered Nurse (RN) license.
- Graduate of an accredited nursing program.
- At least 1 year of acute care nursing experience preferred.
- Infusion therapy experience strongly preferred, including IV therapy, PICC lines, central line care, biologics, TPN, IV antibiotics, and hydration therapy.
- Home care experience preferred but not required.
- Strong communication, organizational, and critical thinking skills.
- Ability to work independently in varied home environments.
