Responsibilities
- Provide case management to help clients overcome barriers to long-term unemployment/underemployment and achieve family stabilization using a Family Development strengths-based approach.
- Triage caseload to ensure adequate support for families in crisis and those with high needs.
- Complete comprehensive assessments of family strengths and needs using agency tools and best practices.
- Collaborate with families to develop goals and milestones using the Brief Solution Focused treatment model and Family Development principles, in conjunction with the Employment Services Team.
- Conduct home visits and interact with clients at off-site locations (e.g., SCCC, DSS) to support task completion, graduation, advancement, and employment retention.
- Regularly evaluate progress and modify goals to support successful completion of training and employment.
- Work independently and effectively communicate and collaborate with team members, both internally and through case conferencing.
- Ensure comprehensive access to SCAP's services by collaborating with agency colleagues and community partners to integrate and coordinate family services.
- Advocate for families in a solution-focused manner and represent SCAP professionally.
Requirements
- Bachelor's Degree in social work, human services, or a related field; Master's Degree preferred.
- Two years of experience providing related services to families and individuals; one year of experience required with a Master's degree.
- Experience in evidence-based practices preferred.
- Knowledge of community-based services, public assistance, and other community resources, with demonstrated skills in accessing these services.
- Ability to quickly learn and utilize resources in Schenectady County and make appropriate referrals.
- Demonstrated written and verbal communication skills.
- Demonstrated ability to perform job responsibilities independently.
- Valid New York State driver's license, regular access to a car, and willingness to travel to off-site locations.
- Demonstrated computer expertise with Microsoft Office and ability to stay current with relevant software.
- Background check required through the NYS Central Registry of Child Abuse.
- Knowledge and understanding of issues related to individuals and families in poverty.
- Ability to build upon the strengths of diverse populations to provide high-level services and lasting solutions.
- Willingness to work a flexible schedule to accommodate family needs.
