About the Role
Service Coordinators are needed for the Early Intervention Program in New York City. This role ensures eligible children aged 0-3 and their families participate successfully in home-based and/or community settings.
Responsibilities
- Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
- Ensure IFSP outcomes and strategies reflect family priorities, concerns, and resources, and adapt to changing needs.
- Coordinate the provision of all necessary services for the child.
- Facilitate the timely delivery of services.
- Continuously seek appropriate services and situations that benefit the child's development for the duration of their eligibility.
- Perform related duties as required.
Requirements
- Bachelor’s Degree in health and/or human services field.
- Bilingual preferred.
- Ability to effectively communicate with clients and families via phone and email.
- Previous experience preferred.
- Case management experience preferred.
