About the Role

Girling Health Care of NY seeks an experienced and reliable Speech Therapist to conduct home visits in Manhattan and Brooklyn. The role involves providing services to patients based on a physician's plan of care, including assessment, evaluation, and participation in developing the total plan of care.

Responsibilities

Assess patient’s speech and language abilities and deficits.
Prepare an initial assessment of the patient’s condition and perform periodic reassessments.
Develop a plan of care in accordance with the authorized practitioner’s plan of treatment for therapy services and according to agency policies.
Provide rehabilitative services for speech and language disorders to meet established treatment goals.
Select and use appropriate diagnostic and therapeutic techniques and materials.
Document type of treatment and patient’s response on designated clinical progress notes.
Complete cases conferences, prepare discharge summaries of service, and complete documentations on appropriate forms.
Participate in the review, revision and implementation of patient care policies and procedures relating to speech-language pathology.
Instruct other health care personnel, the patient and other care takers in methods to assist patient in home therapy programs.
Participate in the development and periodic revision of the plan of care forms.
Collaborate with other health care team members on an ongoing basis.
Participate in inter-disciplinary and case conferences, staff meetings and in service programs.
Participate in the agency’s Quality Assurance Performance Improvement/PIP as assigned.
Demonstrate sound judgment by taking appropriate actions regarding suspected violation of corporate compliance regulations.
Report all suspected violations to supervisor, Compliance Officer or Compliance Hotline.
Perform other duties as assigned.

Requirements

One full year of supervised institutional experience is preferred.
One year home care experience is preferred.
Meets state health status requirements.
