About the Role
Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. On-call rotation required.
Responsibilities
- Provide direct patient care visits for routine provider visits
- Provide symptom management
- Provide Hospice Face to Face visits
- Participate in on-call rotation
Requirements
- Current NH Nurse Practitioner (NP) license
- Minimum 2 years of health care experience
- Reliable transportation
- Valid driver’s license
- Automobile insurance
- Current physical exam
- TB testing
- Required vaccinations
Skills
- Hospice and/or end of life care
- Sympathetic management
- Strong communication
- Ability to collaborate with an interdisciplinary team
- Educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making
